Your health providers will select the health trackers that they will be monitoring in a Remote Monitoring plan based on its objective.
Once subscribed, you will be required to update your monitored health tracker readings as instructed in your monitoring plan. This can be done under the Health Trackers section of your ContinuousCare mobile app or by logging in to your health provider’s Virtual Practice on the web.
You will be able to differentiate between monitored and regular health trackers as the former will have a small icon indicating it is part of a Remote Monitoring plan.

Once you subscribe to a Remote Monitoring plan, only the trackers monitored under that plan will be visible to your doctor. You can update all your tracker readings frequently to track any health variations.
